Wildfire Smoke Kills Tens of Thousands Yearly — Here’s How It Attacks the Body
Smoke from wildfires — which are burning more of the Northern Hemisphere as the planet warms — attacks nearly every system in the human body, killing tens of thousands of people each year, according to a growing body of medical research. As climate change drives more frequent and intense wildfires, scientists are racing to understand the full scope of the health crisis unfolding in the smoke-filled skies above communities across North America and beyond.
The Grim Toll: Lives Lost to Invisible Particles
On average, 24,100 people died each year in the Lower 48 states between 2006 and 2020 due to long-term exposure to tiny particles from wildfire smoke, according to a 2026 study published in Science Advances. On a global scale, the numbers are even more staggering: wildfire smoke particles cause an estimated 677,745 deaths annually, with nearly 39% of them occurring in children under age 5, according to a 2021 study that combined observations, health studies, and computer modeling.
A Stanford University study projects that U.S. wildfire smoke deaths will increase sharply with climate change, potentially reaching an annual cost of $244 billion by midcentury in terms of the economic value placed on each life.
How Wildfire Smoke Invades the Body
The most dangerous component of wildfire smoke is fine particulate matter known as PM2.5 — particles smaller than 2.5 micrometers in diameter. These particles are about 1 micron in size, small enough to bypass the body’s natural defenses, including nose hairs and mucus, and penetrate deep into the lungs.
“If I gave you a list, you would recognize some of these as being very bad, oftentimes associated with the burning of diesel fuel or cigarette smoke, things like formaldehyde or volatile organic compounds,” said Luke Montrose, an environmental toxicologist at Colorado State University. “So just the smoke itself can be bad.”
Scientists have identified at least 1,000 toxins in wildfire smoke, including formaldehyde and volatile organic compounds. Once inhaled, the particles trigger the body’s immune system to mount an inflammatory response, sending signals that can affect nearly every organ.
Immediate Health Impacts: From Asthma to Heart Attacks
The effects of smoke exposure are swift. Within hours, asthma cases spike and ambulance runs increase. Within a day, emergency rooms become swamped with patients suffering from heart attacks, cardiovascular issues, lung problems, and even mental health crises.
Dr. Courtney Howard, an emergency room physician and chair of the Global Climate and Health Alliance, described the acute impacts based on her experience in Yellowknife, Canada — a region warming at triple the global rate. “We found a full doubling of emergency department visits for asthma and about 50% increase in pneumonia,” Howard said.
Smoke also harms pregnant women, increasing the risk of premature births and low-weight babies who may face lifelong breathing problems, according to studies.
Long-Term Dangers: Dementia, Cancer, and Chronic Disease
Beyond the immediate crisis, prolonged exposure to wildfire smoke is linked to some of the most feared health conditions. A 2024 study of 1.2 million Kaiser Permanente members published in JAMA Neurology found a connection between long-term wildfire smoke exposure and increased dementia risk. Research published in the Journal of the American College of Cardiology in February 2026 linked cumulative wildfire smoke PM2.5 exposure to cardiovascular risks, including stroke and heart disease.
A Harvard study published in June 2025 found that health effects from wildfire smoke can persist up to three months after exposure ends, well beyond the few days identified in previous research. The study showed increased risks for ischemic heart disease, arrhythmia, hypertension, pneumonia, and chronic obstructive pulmonary disease.
“More and more studies coming out finding that there’s all types of impacts that may not have been so obvious before,” said Dr. Mary Johnson, an environmental health scientist at the Harvard School of Public Health.
Climate Change: Fanning the Flames
The connection between climate change and worsening wildfire seasons is direct and well-documented. So far in 2026, more than 5,740 square miles of the United States has burned — 31% more than the average of the previous 10 years on this date, according to the National Interagency Fire Center. The amount of U.S. land burned annually in the 2020s is now more than double what it was 30 years ago.
“Wildfires are becoming more frequent and intense because of climate change, and when a fire happens, you have smoke,” said Colleen Reid, a University of Colorado geographic health professor.
Europe saw a record high amount of land burned in 2025, Canada has experienced several record or near-record fire years in the 2020s, and the Arctic has recently seen unprecedented levels of burning.
What Can Be Done?
Public health officials recommend a multi-layered approach to protection. In June 2026, federal agencies including the EPA, CDC, and U.S. Forest Service released an updated Wildfire Smoke: A Guide for Public Health Officials, providing evidence-based recommendations.
Experts advise staying indoors when air quality is poor, using high-quality N95 or equivalent masks when outdoors, and ensuring indoor air filtration systems are in place. Creating clean air shelters in public buildings and monitoring air quality indices through resources like AirNow.gov are also critical strategies.
“Staying away from the smoke is No. 1 if you can,” Johnson said.
The Road Ahead
As wildfires continue to intensify, the health burden is expected to grow substantially. The Stanford projection of 70,000+ excess deaths per year by the 2050s represents a potential tripling of current mortality. The emerging research on cognitive effects, reproductive health, and cardiovascular damage suggests that the full health impact of wildfire smoke may still be underestimated.
For policymakers, the message is clear: climate mitigation and adaptation are not just environmental issues — they are public health imperatives. Investments in early warning systems, indoor air filtration infrastructure, and community preparedness will be essential to protecting vulnerable populations in an increasingly fire-prone world.
